Mostly a general practitioner. He can't really do any major surgery like brain tumors and such. He mostly concentrates on his family. He does everything he can to make sure they know he is there for them. Even though he frequently annoys them they always have him around. Although on the surface Isshin seems like somewhat of a buffoon. And some might think he's a bit off his rocker. I think it's more likely that he's living every day as if it were his last. He knows how quickly someone can be taken away. He's seen it with his wife. He sees it every day in the patients he works around. Isshin is a man who is happy, but a little desperate too. If he goes tomorrow he seems to want to be able to go with no regrets. He's lived his life to the fullest. We also see that he values life in the way he handles tragedy as his job as a doctor. He works very hard to save people and tends to get angry and frustrated that every one around him isn't doing the same. Isshin is a clown dressed up as a man.
